Toxic pollution threatens the health and safety of our families and communities.  Research shows that many of the illnesses our communities face are linked to exposure to toxic chemicals.

The Community Health Action Project seeks to publicize communities across the country where residents believe that environmental factors have led, or could lead to health problems. By filling out the following questionnaire we can share our stories and our strategies for action, as well as unite to fight for a healthier and safer world. Please complete the form below and then click the "submit" button at the bottom of the form:

Please use the following questions to tell your story if they are applicable and helpful.

Please describe how the problem in your community came to your attention, including any health problems experienced in your community, particularly what kinds of symptoms people are experiencing and how many people are affected. How long have these problems been happening?  Have there been government health studies or other investigations?

Describe the environmental exposures that are suspected to be causing the diseases. Include any of the following relevant information: types and amounts of toxics used, stored, dumped, or emitted on site; accidents on site; and impacted populations.  Are there any efforts by the owner or operator to clean up (or avoid cleanup) of the site?

What has your community done to address the problem?  Has an organization been formed to specifically work on these threats to your community’s health?  How many people are involved?  What are your goals?  What actions have you taken to achieve these goals?  What has been your group’s biggest accomplishment?

What has the government done to address the problem?  What government agency(ies) oversees your issue?  What have they done/not done to address your issue?
